Description
The Case Manager will work under the supervision of the Program Director providing a range of support and services to residents living at 219 Henry Street Supported SRO. The goal is to enable and encourage wellness and self-sufficiency. The scope of services will assist each resident to live safety and independently as possible. The Case Manager will promote independent living by providing structured case management services.1. Maintain a caseload of 22 residents. 2.Develop a one-on-one therapeutic relationship and will be responsible for weekly contact with the client. 3.Complete Intake/monthly Summaries, Service Plans and progress notes 4.Assist residents in obtaining and maintaining appropriated medical, psychiatric,and substance abuse treatment. 5.Assist residents in obtaining and maintaining entitlements, e.g. SSI, Public Assistance, Medicaid, housing and employment 6.Assist residents in establishing and/or maintaining family and social contacts 7.Assist residents with financial counseling and medication monitoring.8. Provide crisis intervention services as needed and maintain case records. 9. The Case manager also will be responsible for monthly case notes, room checks, floor and community meetings.10. Escorting residents for entitlements, medical and psychiatric services when needed.

Skills
At least 2 years of case management or related experience working with the homeless, mentally ill and individuals with co-occurring disorder, be familiar with Micro Soft Work and email.
Interested candidates must be able to, read, write and speak English and communicate effectively in oral and written form with clients, family members, agency staff and other service agencies and the general public as well as strong listening skills.
