Job Summary:
The Integrated Health Coordinator will be Sertoma’s lead clinical liaison to managed care organizations (MCOs), Integrated Health Homes (IHH), The Illinois Health Practice Alliance (IHPA) and will ensure compliance with Healthcare Effectiveness Data and Information Set (HEDIS) measures. This position will oversee the care coordination team which will include a Registered Nurse, Occupational Therapist, Care Coordinator, and Case Manager for all Colbert Class Members who are assigned to Sertoma Centre’s Prime network. They will provide direct clinical supervision for team members; and is responsible for the day to day operations of care coordination services implemented throughout the organization. The Integrated Health Coordinator will develop and monitor performance indicators and identify trends and areas of opportunity to proactively resolve operational issues and improve consumer and member outcomes. They will provide and ensure the team provides client-centered, strengths-based services to consumers.

Position Responsibilities:
Organize and direct care coordination team efforts across other departments and programs to ensure standards are met. Communicate with various partners and stakeholders across the organization.
Identify member needs relative to medical and behavioral health through consultation and supervision with Integrated Health Team and develop practices to identify and fill gaps in member service.
Direct responsibility for management of Sertoma Centre’s IHPA Program. Conduct in person and telephonic interventions with members pursuant to tasks assigned using Health EC and Patient Ping databases; delegation of IHPA tasks to Integrated Health Team Members to ensure maximum performance standards as set forth in IHPA program guidelines and communications; lead participant in all IHPA conferences, calls and meetings; ensure that IHPA services are properly billed pursuant to Medicaid. Develop standards of practice relative to the delivery of IHPA related services.
Enhance tracking systems to find those disconnected from treatment through review of claims and initiate contact via Integrated Health Team.
Monitor team workload and production through reports. Provide effective supervision and coaching to all assigned team members, and implement corrective action as needed.
Ensure class members transitioning out of nursing facilities receive necessary care coordination services prior to their move.
Evaluate program’s effectiveness in meeting deliverables. Recommend modifications in service structure or operations that affect the program. Develop plans to improve program performance.

Master’s degree in human services; attainment of LPHA license within 6 months of hire.
Knowledge and/or experience in mental health services required, experience in integrated health preferred.
Minimum of one year’s experience working with individuals with psychiatric disorders and working knowledge of the recovery model preferred.
Three years of case management, care coordination, linkage, outreach, and/or community support experience preferred.

Ability to work in a variety of environments and willingness to provide services in location most convenient to the individual served.
Valid Illinois driver’s license and documentation of current auto insurance, with a good driving record and private transportation available.
Employee is required to remain compliant with all state/federal, position, agency, accreditation, and funding source training/licensing requirements including but not limited to CPR and CPI.
