Position Summary: The Family Team Conference (FTC) Coordinator will report to the Safe Way Forward (SWF) Program Supervisor, and will work closely with all program staff to schedule and facilitate FTCs, coordinating meetings with providers to review the risks and protective factors of a case as well as service goals and next steps. FTCs will take place initially and throughout the life of the case. The FTC Coordinator will also gather reports on occurrence for the purposes of Quality Assurance (QA) compliance.

•Facilitate Family Team Conferences (FTCs) for families in the SWF program
•Oversee the scheduling and coordinating of FTCs
•Liaison with ACS units
•Prepare statistical reports with regards FTC standards and policies
•Participate in Administrative team meetings, staff training, and planning sessions
•Document FTC outcomes in Connections
•Document all conferences in PROMIS
•Liaison with the program’s QA Manager to ensure the program meets all conferencing mandates
•Track, record and coordinate all FTCs for two program sites
•Participate in and conduct ongoing trainings for staff on FTCs
•Including other as needed tasks

•Licensed Master of Social Work plus 2 years-experience post-MSW
•Bilingual English/Spanish required
•Thorough working knowledge of ACS Preventive Services system
•Thorough working knowledge of ACS domestic violence protocols for FTC
•Excellent oral and written communication skills
•Excellent organizational and management skills
•Flexibility and ability to work under pressure
•Ability to connect and have empathy for parents and teens
•Experience with families in crisis and at risk of child placement due to neglect/abuse
•Knowledge of ACS Connections and PROMIS systems
•Trained in the FTC model with knowledge of changes in the Conferencing Policy from fall 2016 through present
