The Case Manager offers support services to clients to increase self-efficacy, self-advocacy, basic life skills, coping strategies, self-management of biopsychosocial needs, benefits and resources, and reintegration into the community. Working within the Medication Assisted Treatment Team, the Case Manager coordinates services and/or resources for clients enrolled in the MAT program, to support their basic needs and enable them to remain and function optimally in the community. The Case Manager will provide marketing and outreach to local clinics, treatment centers, hospitals, urgent care centers, and emergency departments.
The Case Manager works in collaboration with other members of the multidisciplinary team to ensure best possible treatment outcome for the client and provide treatment in accordance with HealthRIGHT 360’s philosophy, goals, policies, mission and vision. The Case Manager will also work with various community partners to promote MAT services. Services may be provided in: outpatient office, residential treatment, or the field, based on client and program need. This position will work with all the LA County Behavioral Health substance use disorder (SUD) programs, including Pasadena, Pomona, and Resolve Recovery.
Provides intensive case management and psychosocial support to those entering the MAT Program to ensure engagement, retention and better treatment outcomes.
Navigates patients through treatment including reminders for appointments and medication management.
Assists the client with intake by completing case management assessment and entering financial and benefit information.
Collaborates with outside agencies to offer referral options, including participating in Learning Collaboratives and presentations.
Completes GPRA data with clients and submits it timely.
Marketing and outreach to other organizations and clients regarding MAT services.
Prepares the client for job search by assisting with resume writing and job search preparation.
Monitors client's progress toward achieving case management treatment plan goals and provides treatment plan input.
Seeks clinical consultation throughout the treatment episode and when encountering new and/or high risk clinical circumstance.
Works collaboratively with other members of the treatment team including the MAT Provider, Clinical Therapist, Counselor, Medical, and Psychiatrist.
Registration or Certification as Substance Abuse Counselor from an approved/accredited California agency.
Bachelor’s Degree from an accredited university.
Preferably a minimum of two years’ experience in the human service field and demonstrated expertise in substance abuse treatment, relapse prevention, and recovery.
Experience working with clients experiencing acute withdrawal from substances.
Knowledge of and experience with providing trauma informed services.
Experience delivering evidence-based practices.
This position will be required to travel to the 4 various SUD program sites.
CDCR fingerprint clearance.
Must not be on parole or probation for a minimum of two years.
A valid California Driver’s License and an insurable driving record under the agency’s liability policy, including reliable transportation.
Skills and Abilities Required:
Professionalism, punctuality, flexibility and reliability are imperative.
Bilingual in English and Spanish (speaking, reading, writing).
Strong computer skills, including Outlook, Excel, Word and PowerPoint.
Excellent verbal, written, and interpersonal skills.
Integrity to handle sensitive information in a confidential manner.
