Wheaton Youth Outreach (WYO)
Wheaton Youth Outreach is a community-based youth and family resource center serving the greater Wheaton area. Services include counseling for individuals, couples and families, crisis intervention for runaway and homeless teens, casework services for basic needs, and transitional housing for young women.
The Lead Case Manager will work closely with volunteer house parents in a transitional housing program, foster families and Outreach staff to provide supportive housing to young adults and help them launch into independent, productive lives. Responsibilities include:
Case management with each resident to assess, plan and implement changes needed for future success and independent living.
Maintain and develop relationships with foster host homes within the community to offer more housing options for homeless young adults.
Work with at-risk homeless youth and young adults in the community to assist with basic needs and steps toward a more secure future.
Ensure compliance with DCFS licensing standards and grant requirements.
This position reports to the Director of Wheaton Youth Outreach and participates as a member of the broader transitional housing and Wheaton Youth Outreach staff teams.
Full-time. Monday – Friday, 8:30 am to 5:00 pm. One evening each week for team meetings; occasional other events and weekends to meet client needs.
IL Driver’s license, reliable transportation and good driving required. Proficiency in MS Excel/ Outlook/ Word. A commitment to OCM’s Statement of Mission & Faith. Bilingual (Spanish/English) a plus.
