The Crisis Outreach QMHP is responsible for providing crisis intervention support on site through a mental health crisis center, and in the community via a mobile crisis unit. The QMHP will outreach to consumers and other community providers to share information about available supports and services. This position will also be responsible for maintaining knowledge of available community resources and providing linkage for consumers to available appropriate resources.
Provide crisis intervention, brief supportive therapy, and/or psycho-educational services utilizing best practice approaches to treatment.
Perform risk assessments as necessary for consumers seeking crisis intervention services.
Assess needs and provide resources and linkage to consumers seeking crisis support services.
Work with community partners including hospitals and law enforcement to ensure continuity of care for consumers when applicable.
Take an active role in learning the best methods to engage in crisis intervention and outreach and suggest changes to program structure as necessary.
Provide outreach and crisis intervention services in the evenings and weekends on a rotating schedule.
Ensure all service documentation is completed in accordance with grant guidelines, state requirements, and organizational requirements.
Qualified Mental Health Professional (QMHP), with Master’s degree in human services related field.
Substance use background or certification preferred.
Previous experience providing outreach and crisis intervention services preferred.
Valid Illinois driver’s license and documentation of current auto insurance, with good driving record and car available.
Employee is required to remain compliant with all state/federal, position, agency, accreditation, and funding source training/licensing requirements including but not limited to CPR and CPI.
Employee is required to possess the skill and ability to utilize all applicable technology and computer programs related to his/her position.
