The Brook Supportive Housing Program has multiple opportunities for Clinical Coordinators to join an established clinical team. The Brook permanently houses 120 special needs tenants, the majority are formerly street homeless and have serious mental illness, chronic illness (including HIV/AIDS) and substance abuse challenges. Clinical Coordinators provide psychosocial support, medication monitoring, financial management assistance and supportive case management services to single adults in a community environment.

Ideal candidate is able to demonstrate ability to work in partnership with clients from a strengths-based perspective, overcoming stigma, and navigating complex systems. Additionally, ideal candidate can adapt to an active environment, is exceptional with time management, has strong writing and communications skills, exhibits excellent attention to detail and remains a team player. Opportunities for group work are also available.

The Clinical Coordinator is responsible for providing programmatic and oversight support to the Program Director which includes supervising a team of social workers and case managers. The Clinical Coordinator is deeply involved in providing direct services to 120 special needs and formerly homeless individuals, that have serious mental illness, chronic illness (including HIV/AIDS) and substance abuse challenges. The Clinical Coordinator will provide clinical supervision to social work staff, contribute to the continued improvement and excellence of the program.


Bachelor's Degree
Licensed Master's Degree in Social Work Required
Minimum two years of work experience
