 WISE & Healthy Aging’s Capability Care Project is a federal grant-funded program that provides intensive, in-home case management services to older adults living alone in the community with dementia, primarily in Santa Monica and surrounding areas of Los Angeles  The Care Coordinator is responsible for providing efficient, thorough, and effective care management services which include a comprehensive assessment of the clients, development of a written Care Plan with measurable outcomes, arrangement and coordination of services, and client follow-up to monitor he effectiveness of the services arranged and assess the need for additional services.  Additionally, the Care Coordinator will communicate regularly with “Volunteer Casual Caregivers” (VCC’s): Highly trained friendly visitors who serve to address both social isolation of client and to be “eyes and ears” for the Care Coordinators between visits.  Supervision for Care Coordinators and VCC’s is provided weekly by the Project Manager, an experienced LCSW.
PRIMARY RESPONSIBILITIES
Conduct client intakes and in-home assessments using program specific forms to identify client needs and confirm eligibility.
Field travel to client’s homes and other sites, as needed and with specific approval from Supervisor (during public health outbreak(s), appropriate PPE will be provided, and physical distancing adhered to for employee and client safety).
Care management/coordination responsibilities including:
Conduct a comprehensive in-home assessment and write a biopsychosocial assessment; Complete paper documentation for all progress notes.
Develop a written Care Plan with measurable outcomes to address and implement an agreed upon action plan for client needs.
Identify appropriate resources and mobilize them to address client needs and resolve problems.
Arrange and coordinate services on behalf of client.
Maintain contact with client to monitor the effectiveness of the service arranged for client and assess the need for additional services.
Write accurate, thorough, and specific client assessments, letters, emails, and other communications.  Complete timely, accurate, and thorough documentation for client files.
Prepare for and participate in weekly Supervision meetings with Manager and other staff.
ESSENTIAL JOB FUNCTIONS
Ability to perform biopsychosocial assessments of clients using appropriate techniques.
Ability to document client information and maintain client boundaries and confidentiality.
Ability to visit clients’ homes with use of personal vehicle or other means of personal transportation.
QUALIFICATIONS
Educational Requirements
Previous care management experience strongly preferred.
Previous experience working with an ethnically and economically diverse population.
Fluency in English (bilingual in Spanish strongly preferred).
Minimum three years of experience working with older adults.
Master’s degree in Social Work, Gerontology, Human Services or related field required.
Other Requirements
Must have reliable transportation, current driver’s license, clean driving record, and proof of auto insurance.
Travel is required within Los Angeles County.
Must pass a criminal background check.
