Job Overview
Prototypes, a program of HealthRIGHT 360 (HR360), provides a comprehensive range of mental health and substance use disorder treatment to people in Southern California, with a focus on pregnant/parenting women. Project Enhancing Access to Comprehensive Healthcare (EACH) will improve integration between behavioral and primary health services for our client population by partnering with a local federally qualified health center (FQHC). Prototypes already provides, offer intensive, individualized care coordination to high-need individuals with severe behavioral health issues. The goal of the Prototypes CCBHC expansion is to integrate primary care and behavioral health services for behavioral health clients receiving Outpatient services at Prototypes’ Pomona Women's Campus.

The Transportation Specialist for the SAMHSA Certified Community Behavioral Health Center Expansion (CCBHC-E) grant is responsible for transporting clients to local designated collaborating organizations identified by the transportation needs of clients. The Transportation Specialist will ensure that clients are safely transported to various locations in order to increase access of primary care and behavioral health care. The Transportation Specialist will use effective communication skills to promote engagement with the client in order to support CCBHC-E objectives.

Key Responsibilities
Transports clients in company-provided vehicle to designated collaborating organizations, including local community health center, in a safe and professional manner.
Communicates with referral source in order to receive relevant data and instructions for transporting clients to appointments.
Understands directions and can read and use maps accurately.
Prepares vehicle usage reports and enters all CCBHC-related transportation activities in company database.
Maintains accurate vehicle and client records in accordance with guidelines established by HealthRIGHT360 and follows all applicable federal and state driving laws.
Maintains frequent communication with the treatment team and develops improved transportation processes.
Cleans and disinfects the vehicle as per COVID-19 and sanitation guidelines and keeps the vehicles maintained and serviced.

Education and Knowledge, Skills and Abilities
?Education and Experience Required:

High School Diploma or GED
Valid California Class B Driver’s license
Minimum 2 years professional driving experience
Background Clearance Required:

Live Scan fingerprint clearance
Must not be on parole or probation for a minimum of two years
5 years DMV printout of no driving violations
