The Client Resources Coordinator III (case manager) will be assisting single mothers with children experiencing homelessness by assessing the needs of families and linking them with services offered by the McGill Transitional House Program and help them transition back to stable housing. The goal of the McGill Transitional House Program Client Resource Coordinator III is to streamline a processes through which homeless families access stable housing.
The position may provide leadership to other Client Resources Coordinators and will handle more difficult cases in serving clients through counseling, advocacy, and guidance. CRCIII is responsible for triage screening, coordinating crisis housing, connecting families to supportive wrap around services to address any immediate needs such as rapid rehousing services, medical, mental health, or work-force development.
•Assesses clients’ needs and helps them address and achieve their identified goals.
•Develops problem-solving relationship with clients.
•Assesses and identifies needs and abilities of clients.
•Develops Housing Stability plans and individualized Service Plan.
•Works with other agencies and the CES-SPA3 to ensure clients receive all needed services.
•Links clients to existing Agency and community resources, including Legal aid.
•Must be able to perform problem solving strategies that prevents homelessness at the front door of the homelessness response system by helping people identify immediate alternative housing arrangements and, if necessary connecting them with services and financial assistance to help them return to permanent housing..
•Records client-related information.
•Compiles monthly statistics of services provided.
•Inputs all clients and residents into Case Worthy and LA-HMIS.
•Attends weekly and monthly case management meetings.
•Monitors cases and plans for re-assessment biweekly.
•Maintains confidentiality of client information and records.
•Master’s degree preferred, Bachelor’s degree required
•Three to five years of related experience to include case management
•Experience working with the homeless population
•Skilled in performing comprehensive case management services, including assessment, service planning, coordination, and documentation.
•Must have a valid California Driver’s License, Class C, Auto Insurance, and willingness to travel as required.
•Ability to use independent judgment and initiative.
