Description
Support people to maintain their independence in the community through the development, maintenance, or accommodation of functional daily living skills.

Position Responsibilities:
Essential Functions
CMHC
Directly deliver services in accordance with applicable Rule 132 program requirements.
Schedule and document all billing activities to meet departmental deadlines, time frames, and productivity standards.
Maintain proficiency using the most current information and technology systems available to function efficiently within the department.
Independently transport consumers in compliance with agency policy and procedure.
Community Services
Independently provide services with consumers in a natural environment.
Spend 60% of their time providing services off-site.
Respond to crisis calls in the evenings, weekends, or holidays as assigned.

ADDITIONAL JOB RESPONSIBILITIES:

Position Specific
•        Develop an assessment to evaluate the impact of a person’s mental, physical, or medical impairments on maintaining independence in a community setting.
•        Assists the team in the development of appropriate individual goals and objectives that supports gaining or maintaining independence in a community setting.
•        Provide direct services that support individual development of homemaking skills, activities of daily living, independently transport within community, recreational activities, and social activities through occupational therapy intervention and approaches within a recovery model.
•        Identifies and procures other necessary supports or accommodations required for maintaining independence in a community setting.
•        Manage multiple demands with flexibility in adapting to new challenges while maintaining priorities.

Customer Service
•        Respect rights and responsibilities of persons served and function as an advocate.
•        Exhibit professional conduct and appearance throughout all work environments in compliance with department standards.
•        Maintain open and effective communication with agency and provider staff, family, and others in response to consumer needs.

Recordkeeping
•        Schedule and status billing activities on a daily basis in the Electronic Clinical Record.
•        Complete all billing notes by the end of the business day every Wednesday for the week prior.
•        Adhere to Rule 132 guidelines for completion and standards of clinical documentation.
•        Report and document consumer incidents in the Electronic Clinical Record.

Safety
•        Respond to and manage crisis situations.
•        Report persons that present as a danger to self or others to be entered on the FOID Registry by a Qualified Examiner.
•        Report critical consumer and staff incidents to supervisor immediately.
•        Transport consumers safely in the community in compliance with agency policy.
•        Maintain a safe workplace by following Sertoma Centre’s safety procedures.

Training
•        Attend all assigned training sessions offered and required by the organization.
•        Seek continuing education units to maintain licensure (as applicable) and best practice standards in community mental health services.
•        Participate in at least one training within one year of hire on trauma-informed principles and/or practices.

Skills
Bachelor’s Degree in Occupational Therapy.
1 year experience working with individuals with mental illness.
1 year case management, linkage, outreach, and/or community support experience preferred.
Experience with Medicaid Rule 132.
Current license in Occupational Therapy.
Valid Illinois driver’s license, auto insurance, a good driving record and car available.
Eligible for certification process to drive PACE vehicles.


Notes
•        Must be willing to work evenings/weekend hours as necessary and be on call to respond to emergencies.
