DIRECT SUPPORT PROFESSIONALS (DSPs) – RESIDENTIAL PROGRAM

We are looking for dedicated individuals to join our team! We have immediate openings available in multiple locations!

This position is responsible for the day-to-day implementation and facilitation of services to individuals residing in the CILA homes they are assigned to, ensuring services are delivered in a person-centered manner and is a member of the Community Support Team for each individual residing in the home.

Our DSP/Service Facilitators provide 24-hour support and supervision to individuals with intellectual and developmental disabilities living in a shared home in local communities. With support of DSP/Service Facilitators, the individual is empowered to take ownership of their home, participate in their local community, and learn skills that promote independence.

Position Responsibilities:
Provide supervision and guidance for consumers during skill building activities.
Develop and implement Personal Plan (PP) for consumers; assists consumers with goals and objectives as stated in their PP.
Document the implementation of the goals, objectives, and service objectives.
Assist with daily living activities, including preparing meals and assisting with personal hygiene.
Develop a rapport with consumers and cultivate a safe and supportive relationship.
Update consumer case notes on a daily basis.
Ensure an appropriate staff-to-consumer ratio at all times.
Transport consumers in CILA van as needed to take individuals to various activities and appointments outside the CILA.
Oversees the consumers’ self-monitoring of medication and passes medication to individuals after being trained by RN.
Works with consumers and other program staff to keep the CILA and its grounds clean and to do other household chores as necessary.
DSPs must be willing to work in other homes as deemed necessary by CILA management staff in order to provide the best quality care to all individuals served.

Skills
Job Qualifications:
High School Diploma or GED recognized by the Illinois Department of Human Services required.
Demonstrated 8th grade reading level.
Valid Illinois driver's license with good driving record.
Completion of all required DSP training within 120 days of hire and maintenance of required certifications.
Minimum of one year experience working directly with individuals with developmental disabilities prefer
