Community Health Worker – Prevention in Practice -- Full time – Fluent in Haitian Creole
BNHC is a multicultural organization that collaborates with community agencies and residents to provide high quality comprehensive health care. We are committed to health promotion and disease prevention. BNHC began providing services in 1994 in a mobile medical van operating out of a church parking lot. Since then, the center has rapidly grown to provide desperately needed services to low-income and marginalized populations in the greater Brockton area. BNHC is committed to providing care that is responsive to community health needs and is linguistically, culturally and financially accessible. BNHC serves an extremely diverse patient population including Brazilian, Cape Verdean, Haitian, Latino, Portuguese, African American and Caucasian, 77% of whom live in poverty.
BNHC is looking for a Community Health Worker who can break down barriers and use their experience and knowledge of the community to be a change maker. A Community Health Worker is a person who utilizes language and cultural experience to serve patients by building bridges between patients and community resources, as well as providing health education to meet patient needs. The Community Health Worker provides outreach, support and health education to patients who have pre-hypertension/diabetes, diabetes and hypertension. The CHW works with all partners in our “Prevention in Practice” program to build and support a healthy living environment. The outreach, patient support and health education will occur in many different locations and meeting formats. The goal is to be a liaison between providers, partners and facilitate access to services and to improve the quality and culture of service delivery.
