Behavioral Health Community Partnership Case Manager (BHCP CM) -- Full time

Since then, the center has rapidly grown to provide desperately needed services to low-income and marginalized populations in the greater Brockton area. BNHC is committed to providing care that is responsive to community health needs and is linguistically, culturally and financially accessible. BNHC serves an extremely diverse patient population including Brazilian, Cape Verdean, Haitian, Latino, Portuguese, African American and Caucasian, 77% of whom live in poverty.
BNHC is looking for a Behavioral Health Community Partnership Case Manager (BHCP CM) who can break down barriers and use their experience and knowledge of the community to be a change maker BHCP CM works in collaboration and continuous partnership with chronically ill or “high risk” patients and their family/caregiver(s), clinical/hospital/specialty providers and staff, and community resources in a team approach.

Candidates must have 1 Year of experience working with mental health/substance use disorders with a BA in Social services field or 2 Years of experience working with mental health/substance use disorders with Associates, or CHW Training Certificate. Bilingual Required in CVC, Portuguese, Spanish, or Haitian Creole.
