Description FORENSIC TREATMENT AGENCY seeking to fill positions of Case Manager to work as part of a multidisciplinary team. The positions will be based in Downtown (3rd & San Pedro) and Hope on Alvarado (Beverly and Alvarado)

Duties will include:

* Individual case management
* Assist with crisis interventions
* Provide life skills and/or individual rehab services
* Accompany and provide support to clients when accessing services in the community (as needed)
* Maintain and submit billing records in a timely manner (vary by program)
* Maintain and update client files and documentation in accordance with DMH or DHS guidelines
* Running of behavior modification groups (vary by program and/or COVID protocols)
* Participate, coordinate and implement cooperative arrangements and/or team conferences with various inter-disciplinary team members in the development and implementation of client treatment and aftercare plans
* Coordinate services with various public and private community health, mental health, and social service agencies to ensure appropriate services for clients.
* Transport client(s) to connect with necessary community resources
* Optional on-call rotation: Respond to crisis calls for assessment and pertinent intervention.

* Preferred 1 yr minimum case management experience
* Preferred one year of direct service experience in a mental health setting which has an interdisciplinary team approach
* Preferred knowledge of DHS or DMH paperwork
* Preferred experience in offender/forensic treatment
* Preferred knowledge of substance use disorders
* Must be comfortable transporting clients in personal vehicles

* Must have own transportation (Will run a DMV check), valid California Driver License, and proof of automobile insurance coverage
