We have a wonderful opportunity for a Case Manager LGBTQ at TLP & Host Homes Program. Under the supervision of the Program Manager, the Case Manager – TLP & Host Homes for LGBTQ is responsible for providing general case management around education; housing, supportive counseling and connections to needed resources and services to clients identifying as LGBTQ.ESSENTIAL DUTIES AND RESPONSIBILITIES:
• Provide psycho-education, skills based & communication strategies, and peer supportive counseling in 1:1, group modalities to LGBTQ clients in Host Homes and TLP
• Complete all assessments on each youth, SBIRT, Human Trafficking, lifelong connections, well-being, VI-SPDAT and assess youth’s relationship with family and if they would benefit or desire family counseling to support re-connections and possible reunification.
• Develop Needs and Service plan WITH youth in Host Homes and TLP
• Contract with youth weekly and support them in completing agreed upon goals
• Maintain client file, consents, releases of information, emergency contacts, progress notes, needs and service plan, etc
• Enter all clients into AWARDS and CMS and enter data on all services received, progress notes, assessments, etc.
• Send Intake, Discharge and annual update information to the HMIS data entry staff.
• Assist with crisis intervention &/or link to On Call as needed
• Provide and/or link youth to safe, pro-social activities
• Provide linkage to mental health services as needed through: psycho-education, referrals,
• Provide HIV/STI and drug and alcohol abuse information
• Provide transportation as needed
• Complete program and statistical forms as required including providing reports as requested.
• Ensure all client data is captured in AWARDS electronic health record within required deadline. Maintain paper and AWARDS files updated at all times.
• Participate in audits to ensure documentation compliance
• Prepare and process all required documents for financial assistance.
• Process financial assistance for move in costs, deposits and/or delinquent rent. Follow agency policies and procedures regarding financial documentation and reconciling all requests.
• Attend trainings and agency meetings as required/appropriate. Attend monthly LGBTQ committee meeting.
• Provide referral services for LGBTQ clients to other BWC or community programs
• Participate in program evaluation and recommendations.
• Coordinate Focus Groups with LGBTQ clients and provide reports on findings
• Provide recommendations for LGBTQ specific, complementing programs and services that can be added to BWC’s offerings
• Attend community meetings as appropriate.
• Supervise volunteers that may be assigned to the program.
• Participate in agency Program Quality Improvement initiatives and/or meetings
• Participate in ensuring safe work environment and promoting safety initiatives.
• Maintain TLP house in appropriate conditions at all times and report any safety or other repair issues immediately to the Facilities team.
• Regular attendance is a requirement of the position.
