This position is responsible for the development, implementation, and facilitation of the Implementation Strategy process for persons served, ensuring integration of service planning through the involvement of the individual, family, guardian, other service providers, agency staff, and others identified by the individual.


Develop and implement Implementation Strategy for consumers; assists consumers with goals, outcomes, and objectives as stated in their Personal Plan (PP).
Participate in Discovery Tool process, complete Risk Assessment, ICAP and BWAP, as necessary, prior to scheduled annual meeting in order to determine strengths, abilities, needs, interests, preference, and choice in service provision.
Work cooperatively with staff and provide direction and training to staff, students, volunteers as needed for Implementation Strategy, data collection, Implementation Strategy revision, and to ensure appropriate access to services as identified in Personal Plan.
Complete all required agency management reports, billing, personnel documents, and others as required in a timely and accurate manner.
Ensure delivery and/or coordination of all services in compliance with DHS Rule 119, CARF standards, agency mission, agency policy and procedure, program guidelines, and best practices.

Minimum of one year experience working with individuals with developmental disabilities and/or mental illness.
Bachelor’s Degree in a Human Service field of study required.
Certification by DHSA as a qualified QIDP.
Effective communication skills, both written and oral.
Valid Illinois driver’s license and documentation of current auto insurance, with a good driving record.
