Hudson Guild is looking for a dynamic case manager to join our Social Services team! You will be responsible for providing case management services to a diverse population of formerly homeless adults living in the Chelsea and Clinton areas of New York City. If you are high-energy, organized, engaging and can foster relationships with residents in independent supportive housing, this is the job for you. Under the direct supervision of the Assistant Director of Social Services, you will join a team of six case managers who work collaboratively while still having the ability to work independently.
REQUIRED QUALIFICATIONS: BA or BSW in related field (with emphasis in Social Services). Bilingual in Spanish a must.
Provide concrete services (benefits and entitlements)
Provide individual counseling, support, advocacy, resources and referrals
Utilize a variety of engagement and treatment modalities
Maintain appropriate documentation and treatment planning, and work collaboratively with fellow staff/psychiatric consultations
Conduct new tenant orientations
Assist tenants with relocation efforts as necessary
Attend tenant and staff meetings
Participate in tenant selection committee as requested
Produce monthly reports
Participate in case conferences
