Esperanza {HOPE} Counseling Services provides in-home counseling services to home-bound seniors who suffer from moderate depression. The program’s focus is to enhance the mental and emotional well-being of our clients. The Mental Health Counselor will provide in-home assessments and weekly counseling services to clients referred through RB Senior’s Case Management program. Eligible clients will need to meet the criteria of suffering from moderate depression, as assessed by their Case Manager, utilizing the PHQ-2/9.

The Mental Health Counselor will:

- Conduct intakes in the home with clients referred by RB Case Managers, including assessment of client’s depression severity through the use of the PHQ-2/9.

- Conduct weekly counseling sessions in the client’s home.

- Establish service goal plans with clients that include development of positive coping mechanisms and ways to alleviate stress and decrease anxiety.

- Provide education on other therapeutic treatments and/or interventions that may help client attain overall well-being.

- Collaborate with the Program Director to ensure client has access to auxiliary services they have expressed need for during their counseling sessions.

- Participate in case conferences during in-office supervision with Program Director to ensure clients are connected to appropriate services and assess programmatic goals and outcomes.

- Complete and maintain documentation according to all applicable guidelines; record case notes for each session in the AWARDS.

- Conduct follow-up assessments quarterly to determine improvement in client’s PHQ-2/9 score.


- New York State Licensed Master of Social Work

- 2-5 years of experience providing counseling services within a clinical setting

- Dynamic professional with working knowledge of treating depression and anxiety as well as unique challenges facing the aging population

- Excellent written, verbal, and interpersonal skills

- Ability to travel to multiple sites and work remotely

- Bi-lingual English/Spanish strongly preferred


Contract Employee – 25-30 hours per week


$45/hour for initial visits/assessment visits

$35/hour for ongoing sessions
