New York Foundation for Senior Citizens, Inc. is dedicated to helping New York's seniors enjoy healthier, safer, more productive and dignified lives in their own homes and communities and to help them avoid premature institutionalization.

The Case Management Program, located in lower Manhattan, serves seniors 60 and over who reside in Manhattan Community District's #1, 2, 3, 5 & 6.

The Case Manager's responsibilities include:
*Conduct in-home assessments with the frail elderly (*Due to COVID, in-home assessments are now being done remotely however field work may still be required in the event of an emergency)
*Assist with benefit/entitlement applications
*Complete data entry and referrals
*Monitor client needs and satisfaction with services received
*Conduct outreach in the community as needed
*Assist with intakes as needed

Master of Social Work is strongly preferred.
At least one year of supervised experience in human services is required.

Bilingual Required: English/Chinese, English/Spanish, English/Polish, English/Russian.

Excellent communication, organization, and writing skills required.
