Description
The HARP Client Services Manager is responsible for managing the HARP assessment process from start to finish for clients enrolled in the Health Home program. This includes working with the Health Home Care Managers and their clients to educate them on HARP and Home and Community Based Serves (HCBS). The HCSM is responsible for completing the HARP assessment with eligible clients, completing the Plan of Care, and working collaboratively with the interdisciplinary care team, specifically Managed Care Organizations and HCBS Providers to coordinate services.
The successful candidate will have provided direct services to people with serious mental illness, developmental disabilities, or substance use disorders or have had the responsibility of linking individuals described herein to a broad range of services essential to successful living in a community setting.


Skills
•        Proven ability to empathize with the clients we serve.
•        Tenacity and passion for this work with the ability to balance objectivity with empathy.
•        Computer literacy required.
•        Valid Driver License required and a driving record that will permit the use of an Agency vehicle to transport clients to appointments or other activities as needed.
•        Bachelor’s Degree required.
•        Bilingual English/Spanish a plus.
