Description
Essential Functions of the Role:

Manage workflow for Health Home HARP clients to enroll in Home and Community Based Services (HCBS). Monitor compliance with all policy, procedures, and documentation requirements for HCBS. Supervise HCBS staff and ensure all requirements are met for billing submission.

Educate employees on company policies and procedures regarding access to care, the grievance and appeals process, the eligibility process and Medicaid, PHI as well as to conduct trainings in other areas as needed.


Participate in the review of departmental productivity and performance reports and takes an active part in the development of action plans for agency performance improvement.

•        Oversee HCBS staff, including some aspects of training, hiring, and payroll.
•        Attend ISP meetings for consumers within the program.
•        Conduct quarterly on-site visits to member's home to monitor program outcomes.
•        Ensure ISP outcomes and objectives are written, disseminated, and implemented.
•        Compile GAP reports and fill in for gaps in coverage that are designated by ISP requirements.
•        Ensure authorizations are current and services are provided within specifications.
•        Maintain client files to ensure OMH and Medicaid contract compliance.
•        Keep up-to-date on regulatory and policy changes
•        Participate in the review of departmental productivity and performance reports and takes an active part in the development of action plans for agency performance improvement.

•        Perform other duties as assigned

Skills
• Previous supervisory experience with a Health Home Care Management Agency or OMH program is highly recommended and preferred.
• Must have good initiative with the ability to meet specific department goals.
• Must be an effective leader. Gaining rapport with clients, families, and staff is crucial to the success of this position.
• Valid Driver’s license


Notes
•        Must have a licensure (e.g. LMSW, CASAC, LMHC, LPN)
•        Basic computer skills, superb communication skills, and reliable transportation is a must.
