There are two positions available. One is Site-Based and the other is Without Walls. The Program Coordinators are responsible to develop, implement and coordinate our site-specific Day Habilitation or Without Walls Day Habitation Programs. The Program Coordinator will provide supervision to all staff. Ensure daily program schedule and activities are followed. Manage the coordination of instructional activities, recreational community outings and seek volunteer opportunities. Instruct Supervisors in the development of initial (30-day), Semi-annual and Annual Day Habilitation Plans. Review all Day Habilitation plans related to supporting billable documentation on a semi-monthly basis. Ensure all types of communication are maintained professionally to caregivers, Care Coordinators and all other professionals. Work with the Director on special program related projects to include curricula development, new admissions, group placement, group transfers and staff development. The Program Coordinator will ensure compliance with all OPWDD regulatory requirements, On Your Mark, Inc.’s policies and procedures and maintain the quality of care provided in accordance to the mission of On Your Mark, Inc.

- Comply with all OPWDD and New York State Justice Center Regulations.
- Comply with all On Your Mark, Inc. policies and procedures.
- Ensure the safety and well being of all individuals within the program.
- Provide input and feedback regarding program and activities.
- Assume additional responsibilities when assigned for the safety of the program and quality of care for all the individuals.
- Carry out any other responsibilities concerning programming and safety deemed necessary by the Assistant Director or Director

- Knowledge of intellectual disabilities and individual differences.
- Strong organization and communication skills.
- Able to quickly recognize changes, symptoms or other problems related to a participant’s condition, determine the appropriate intervention and take the necessary action.
- Self directed and motivated to function independently, completing tasks in a timely manner yet remaining flexible to participant’s needs.

- Bachelor’s Degree in a related field preferred.
- Proven supervisory experience in the field of Developmental Disabilities and experience in DayHab setting preferred.
- Excellent written, Interpersonal and communication skills.
- Understanding of curricula and ability to design specified curriculum.
- Knowledge of OPWDD regulations.
- Proven team-oriented skills.
- Valid Driver’s License.
