The Clinician 2 provides youth with counseling, case management and referral services, and preparation for independent living and also provides mentoring for Mental Health Specialist I’s in developing and maintaining quality services to TAY clients.

•        Provide mental health intake & assessment, case management & referral services in Transition Age Youth (18 - 24) program; in-home & on-site
•        Assist youth in developing & implementing an individualized care plan to meet mental health needs, long-term housing & independent living needs; assist with skills-building including parenting skills as well as developing educational & vocational plans.
•        Must meet agency & County of Santa Clara mental health standards & deadlines for all documentation, statistics & reporting.
•        Must be able to efficiently travel to client homes for appts & provide clients with transportation as needed
•        Participate in program crisis on-call rotation. Flexible schedule required.
•        Mentor Mental Health Specialists
•        Attend meetings and trainings as required
•        Participate in Quality Improvement & Safety activities & initiatives
•        Ensure that site is neat and tidy and report all maintenance or safety issues to manger or Facilities Manager immediately.
•        Participate in agency Program Quality Improvement activities and initiatives
•        Participate in agency Safety initiatives and proactively report safety issues to management immediately
•        Regular attendance is a requirement of the position.
•        Other duties as assigned.

MA-4yr college or university; 1-4yrs related exp and/or training; or equivalent combination of education and experience. Knowledge of needs of runaway, homeless and street youth and experience working with culturally diverse youth & families required. Knowledge of SCC Mental Health systems & documentation procedures. Valid CA drivers license with access to a personal vehicle with current Ins. Fingerprinting, TB test&CPR/First Aid required. Bilingual (Eng/Spanish or Eng/Vietnamese) DESIRED.
