The Transgender Health Coordinator works closely with providers, and community members to assist in obtaining care, screenings, and other services for transgender individuals. This position may perform assessments, intake, community outreach, link individuals to care, and provide educational materials to patients. The Transgender Health Coordinator’s goal will be to provide resources and knowledge to patients that will make access to care and services uncomplicated and support each patient in receiving the affirming care they need.


Analytical and Critical Thinking
•        Identify needs of patients and coordinate access to services
•        Receive information requests from providers and act upon those requests

Checking, Examining, and Recording
•        Document interactions both with patients and on behalf of patients to ensure continuity of care

•        Participate in team discussions regarding patient care and access
•        Advocate for the patient both internally and externally as needed
•        Participation in evening or weekend activities to reach at-risk communities
•        Update providers with information as needed related to patient care
•        Provide pre-set materials to patients to inform them of risk and prevention strategies

Problem Solving and Decision Making
•        Identify barriers incurred by the patient and work to resolve them. This may be with internal systems, outside providers, insurance companies, pharmacies, or other community associations

•        Required: Masters degree and 3 years of work experience with the target population
•        Social work license: LCSW-C
•        Cultural competence and knowledge of individuals across varying races and socioeconomic status
•        Understanding of sexually transmitted diseases and experience in educating others in risk and prevention
•        Knowledge of insurance practices and barriers encountered by transgender individuals

•        Excellent verbal and written communication
•        Proficiency with Microsoft Office
•        Ability to navigate difficult conversations with respect, tact, and professionalism
•        Ability to persist to overcome complex challenges and barriers
•        Ability to work as part of a team to achieve a common goal
