In this role, you will be responsible for coordinating, and monitor options and services to meet at-risk patients healthcare needs through communication and coordination with providers to promote quality, cost-effective outcomes and appropriate level and mix of care. You will provide direct social work services, including assessment, intervention, and recommendations to individual patients and their families. Participates in interdisciplinary activities, complies with departmental standards regarding attendance, documentation, continuous quality improvement, statistics, departmental policies and procedures, and the Code of Ethics. Keeps current with professional social work health care knowledge base and pursues further enhancement of job-related knowledge.
Must possess a current license (LCSW) with the California Board of Behavioral Sciences.
A Master’s Degree in Social Work from a school accredited by the Council on Social Work Education.
Excellent oral and written communication skills. Demonstrated skill in psychosocial assessments of patients/families, clinical interventions, and treatment planning with patients and families. Ability to analyze problems and formulate plans, solutions and a course of action given changing circumstances. Ability to be flexible and work according to availability of office personnel, patient and resources required to coordinate care of patient.
