Description
The Case Managers primary responsibilities include:


Coordinate all services necessary to maximize consumer recovery and independence for individuals with mental illness.

Ensure case management compliance with agency standards and the practice model.

Assist consumer to improve quality of life and maintain the highest possible ability to recover and function within the community.

Implement strategies for obtaining services and supports that are goal-oriented and individualized.

Skills
Educational / Talent Requirements:
1. Social Work degree from an accredited institution is required. LLBSW or LLMSW.
2. LBSW or LMSW certificate preferred.

Work Experience Requirements:
1. Prior case management in a mental health or rehabilitation setting preferred.
2. Maintain a valid motor vehicle operator license with acceptable driving record according to Hope Network insurance carrier definitions.
