OVERVIEW OF DUTIES: The In-home Support Counselor (IHSC)/Case Worker will be responsible for reviewing the individual needs and services plan with the ISFC resource parent and child and family team. In addition, the IHSC/case worker will provide case management services to Adoptive and Foster families.

•        Provide in-home direct services to resource families and one-on-one services to the ISFC child/youth as needed. Establish positive and supportive relationship with the ISFC child/youth and his or her family/caretaker and maintain appropriate and professional boundaries.
•        Ensure 24-hour availability and crisis intervention on as needed basis. Participate in the development of appropriate crisis plans.
•        Participate in the development and implementation of behavioral plan and treatment goals with the client and caregivers.
•        Review the individual needs and service plan with the ISFC Resource Parents, the child and the family team as needed.
•        Case management includes: Direct contact with families to facilitate the application and approval process, home visits to assist with the paperwork, telephone calls with county social workers to provide updates about case plan, attend and sign adoption placement paperwork with the family and county social worker, provide family with appropriate community linkages and referrals.
•        Conduct all functions in accordance with agency guidelines, policies and procedures.
•        Maintain communications with ISFC Resource Parents, child, family team and all other stakeholders directly involved in the client’s treatment plan.
•        Perform other duties and responsibilities as assigned.


Education: Bachelor’s in Social Work (REQUIRED).

Experience:        At least six (6) months of experience in working with children who have serious emotional or behavioral needs or children who have special needs including but not limited to intensive medical needs in child welfare.

Knowledge of:        Demonstrated knowledge of working with family programs and providing training.

Language Skills: Bilingual (English/Spanish) required.

Driver’s License: Possession of a valid California driver’s license is required and must be insurable with the agency’s insurance carrier.
