We have a wonderful opportunity for an Outreach Case Manager in our SCC Housing Initiative program at the Drop in Center. Under the supervision and direction of the Program Manager and Program Supervisor at the Drop In Center, the Outreach Case Manager conducts street outreach throughout Santa Clara County and engages homeless minors, young adults and homeless families with young adults as head of households, including those that are unsheltered.

•        Conduct street outreach to areas frequented by street youth throughout Santa Clara County. Use Assessment and Intake forms as required by County and Continuum of Care.
•        Provide youth connections to services, interim housing and permanent housing.
•        Assess services that can be provided at Drop In Center or other BWC programs.
•        If outside referral needed, provide warm handoff to other providers where clients are referred.
•        Ensure 10% of clients assessed are enrolled in transitional housing, rapid rehousing or permanent supportive housing programs.
•        Follow-up with youth placed in safe housing alternatives
•        Maintain statistical data and complete reports as appropriate. Enter ALL client data into AWARDS AND HMIS systems. Must maintain AWARDS and HMIS updated with all client information and data.
•        Attend agency and other program meetings as appropriate.
•        Participate in agency Program Quality Improvement activities and initiatives
•        Participate in agency Safety initiatives and proactively report safety issues to management immediately
•        Attend training as needed/appropriate
•        Ensure that any BWC vehicles used are kept tidy and that any maintenance or safety issues are reported to the Facilities Manager immediately.
•        Ensure that DIC site is neat and tidy and report all maintenance or safety issues to manger or Facilities Manager immediately.
•        Regular attendance is a requirement of the position.
•        Other duties as may be assigned.

AA or one to two years related experience and/or training. Experience working with homeless and culturally diverse youth required. Knowledge of needs of homeless and street youth and experience working with culturally diverse youth and families required. Bi-lingual (English/Spanish) candidates highly preferred. Driving is a requirement of the job (valid California driver’s license with access to a personal vehicle that can be used for company business and current personal automobile ins)
