SUMMARY: Under the direction of the Program Director, the Veteran Outreach Worker is responsible for providing outreach services to homeless mentally ill adults who are veterans, with a focus on establishing linkages between the homeless mentally ill and a variety of services including shelter, social services, medical & psychiatric care, & MHA’s Multi-Service Center.
DUTIES AND RESPONSIBILITIES:
1.Engage homeless veteran’s in a variety of settings such as parks, beaches, food programs, shelters, etc., to assess their need for services.
2.Assist homeless veteran with transportation to medical services, mental health clinics, shelter beds, social service agencies, and offer access to MHA’s Multi-Service Center.
3.Conduct initial assessments in the field to determine program eligibility and identify entitlements and resources that the individual may require and for which they may be eligible.
4.Maintain statistics and other program paperwork in accordance with agency and funding requirements.
5.Attend regularly scheduled supervision and staff meetings.
6. Other duties as assigned by Program Director.
1.Service connected with an honorable discharge and possession of an Honorable discharge DD214.
(a) A Veteran of the Armed Forces.
2.Possession of the following:
(a)Bachelor’s degree in Social Work or related field, or
(b)Sufficient personal experience with homelessness and/or mental illness to qualify the candidate to fulfill the requirements of the position, or
(c)Two years equivalent experience working in a mental health or social service setting.
3.Possession of a working knowledge of entitlement programs, housing and shelter resources, legal services, mental health and substance abuse services, health care resources, vocational training and rehabilitation, and employment assistance.
4.Capable of providing crisis intervention services to address acute psychiatric episodes, health emergencies, or personal crisis.
5.Excellent social and communication skills of a level sufficient to engage consumers, build trusting relationships, and encourage them to participate in the program.
6.Possession of a valid California driver’s license and a driving record meeting agency insurance requirements.
