Description
Arbor Inn is seeking a compassionate and organized Case Manager to support homeless families working toward permanent, independent housing. As part of a two-floor residential team, you'll play a key role in ensuring the safety, stability, and progress of the families we serve — combining hands-on case management with advocacy, group facilitation, and community coordination.


What You'll Do
* Conduct initial and ongoing assessments, service plans, and progress notes in CARES and manual case records, meeting agency and State deadlines
* Complete morning floor rounds to help residents stay on track with their Independent Living Plan (ILP) tasks
* Design and lead groups and activities tailored to residents' developmental needs and abilities
* Facilitate floor meetings and self-government sessions that promote resident independence and accountability
* Advocate for residents' access to benefits and community resources; track and support medical and other essential appointments
* Partner with the Housing Specialist to prepare residents for the transition to independent living
* Participate in staff meetings, in-service trainings, and agency-wide meetings
* Maintain accurate, up-to-date records and reports
* Support overall facility safety, security, and maintenance alongside fellow staff
* Perform other related duties as assigned by the Assistant Program Director and/or Program Director

Skills
*Experience as a Case Manager working with homeless families
*Experience in a residential setting, including counseling and group facilitation
*Solid knowledge of public benefits systems and related resources
*Strong verbal and written communication skills
*Excellent interpersonal skills and the ability to work collaboratively as part of a team
*Covid Vaccination required
