Description
We are seeking Case Managers to support two distinct populations within our building: formerly homeless adults, most of whom have a serious and persistent mental illness, and formerly homeless families. In both roles, you will work collaboratively with staff to ensure the safety, stability, and overall well-being of residents, taking a proactive approach through floor meetings, community engagement, and clinical involvement.


What you'll do (not an exhaustive list):
Core Responsibilities (Both Roles)

*Perform all activities related to case management for an assigned caseload, including timely service plans, reviews, psycho-socials, and progress notes.

*Document activities and submit statistics per government funder and Agency mandates in a timely manner.
Maintain and document liaisons with other agencies providing services to the caseload.

*Develop activities/groups focused on skill building, socialization, activities of daily living, and recovery issues that support resident functioning in community living.

*Facilitate Floor meetings aimed at building community on each floor and negotiating differences.

*Participate in intake interviews with the team, complete interviewing forms, and provide admission recommendations.

*Attend staff meetings, agency meetings, and training sessions as assigned; document and submit training forms upon completion.

*Provide individual counseling and crisis intervention as needed.

*Develop resource connections that can integrate into affiliation agreements for service provision between organizations.

*Assess incidents and coordinate emergency responses involving Police, EMS, Fire Department, Maintenance, and other officials; document and submit all supporting documentation to the supervisor.


Adults Program – Additional Duties

*Assist with tenant/community meetings and special interest speakers.

*Participate in clinical activities, case reviews, and recreational activities.


Families Program – Additional Duties

*Maintain and document liaisons with agencies including ACS.

*Develop age- and developmentally appropriate activities/groups for children and young adults focused on skill building, socialization, recreation, and education; work with Activities staff on tutoring or other educational groups.

*Facilitate parent support groups, support groups for domestic violence survivors, and other family support groups as indicated.

Skills
Families Program

*Experience as a Case Manager or in an equivalent position with at-risk families is required.

*Knowledge of child development and family dynamics preferred.

Adults Program

*Experience as a Case Manager with the mentally ill and/or with homeless adults in a residential setting preferred.
