We are hiring Substance Abuse Counselors for our Psych Rehab program at two residential locations. Core responsibilities are shared across both sites, with some location-specific duties noted below.
What you'll do (not an exhaustive list):
Core Responsibilities (Both Locations)
*Carry an assigned caseload and complete timely documentation, including service plans, quarterly reviews, psycho-socials, and progress notes.
*Document substance abuse services provided during all relevant participant interactions, including individual and group contacts.
*Co-facilitate/coordinate groups and trainings in Relapse Prevention, Crisis Management, Harm Reduction, Peer Counseling, Double Recovery, and Medication Management, empowering residents to assume leadership.
*Work closely with Case Managers, Residential Counselors, the Program Director, Deputy Director, and Case Management Supervisor to ensure an integrated approach to Psych Rehab services.
*Identify and agree upon substance abuse needs and goals with residents within the Psych Rehab framework.
*Teach and encourage positive social behaviors, conflict resolution, and joint planning of community/apartment-centered activities.
*Provide crisis intervention, which may include calling 911 and/or accompanying a resident to the hospital.
Prepare psycho-socials as required.
Pacific Street – Additional Duties
($1,500 Signing Bonus - After 60 days of employment).
*Develop a program-wide resource guide of treatment providers (detox, long-term rehab, MICA programs, etc.).
*Design activities supporting development of independent living skills for residents in recovery.
*Develop an assessment tool for use with intake packets and treatment planning.
*Publish a quarterly recovery newsletter and submit monthly reports on substance abuse interventions and outcomes.
*Counsel and advocate for residents to join advocacy groups/committees addressing substance use issues.
Chapel Street (OMH) – Additional Duties
*Assist Case Managers and residents in writing Admission Notes documenting substance abuse services on move-in day.
*Document substance abuse services during apartment visits per the agency's Tickler System.
*Document Medicaid billable notes and submit for timely review.
*Make regularly scheduled visits to assigned caseloads in their apartments.
*Work with apartment roommates to promote smooth functioning through positive citizenship behaviors and conflict resolution.
