Community Support Team (CST) Leader

About the Role

The Community Support Team (CST) Leader serves in a dual capacity as both a clinical supervisor and practicing clinician and must work a minimum of 0.5 FTE. This role provides clinical and administrative supervision to the Community Support Team while delivering direct clinical services.

The CST Leader is responsible for the planning, coordination, and oversight of therapeutic and rehabilitative services for individuals enrolled in Community Support Team services. This includes leading the development, implementation, and ongoing revision of each client’s Person-Centered Plan (PCP).

The role requires continuous assessment of client needs, coordination of services aligned with the PCP, evaluation of intervention effectiveness, and ongoing measurement of client outcomes to ensure high-quality, person-centered care.

Key Responsibilities

*Provide clinical and administrative supervision to CST staff

*Function as a practicing clinician on the team

*Lead development and ongoing updates of Person-Centered Plans (PCPs)

*Coordinate and oversee therapeutic and rehabilitative services

*Conduct ongoing client assessments and adjust treatment plans as needed

*Evaluate effectiveness of interventions and track client outcomes

*Ensure services align with regulatory and program requirements
