Key Responsibilities & Essential Functions

Program/Facilities

Facilitates, creates, implements, and/or supervises on-site groups geared toward maximizing client readiness for independent living and recovery, including all wellness self-management (WSM), self-advocacy, leadership and activities of daily living (ADL) groups. These groups include, but are not limited to:
Wellness Self- Management curriculum
Physical and mental health awareness
Artistic expression
Housing
Banking
Cooking
Self-care
Physical activities
Coping with mental illness and/or chemical dependency symptoms, etc.   
Facilitates and participates in team meetings with other social service staff to focus on sharing and integrating information related to client recovery and next step housing service delivery, when applicable.
Works closely with Case Managers to coordinate housing resources and related information (OMH transitional housing).   
Generates and submits human resource administration (HRA) housing documentation for clients to housing organizations.
Works with consumers and staff to design and post activities program calendars and flyers/posters for events and activities.
Maintains and stores wellness and recreation equipment and supplies.
Monitors and analyzes participation and outcomes of activities and determines future planning.
Communicates with and assesses clients looking for housing admission with applicable representatives.
Inputs external submitted HRA documentation into the client database system(s).
Reports potential client program eligibility to supervisors.
Researches, develops and presents monthly health initiatives highlighting awareness, education, prevention and wellness to clients.
Attends client-facilitated monthly advocacy meetings and inputs notes into client database system(s).   
Works with clients and staff to design and manage monthly activities (events, groups, workshops, etc.), and creates, advertises and distributes monthly site calendars, flyers, posters and information regarding activities.
Analyzes and records client participation and activity outcomes to assess program workability and needs.
Monitors client self-administration of medications, as required.
Administrative

Maintains and submits all necessary documentation and reports accurately in a timely manner and as required by Urban Pathways, regulatory and funding agency policies and procedures including but not limited to:
Enters progress notes after each client meeting within 48 hours and/or as mandated by the Program.
Completes Intake/admission documentation within 24 hours of client admission, as mandated by Urban Pathways, regulatory and funding agency policies and procedures, when applicable.
Documents assessments in client records within required timeframe (not applicable for Outreach Program).
Updates information in client records, as required.
Documents incidents and distributes related notices within 24 hours when the Wellness Self-Management Coordinator has witnessed the incident.
Submits all required paper work for supervisor approval within 48 hours.
Files paperwork in client charts (once approved/signed by supervisor), as required by program (no more than 48 hours).
Compiles and reports monthly statistics in various data bases and generates progress notes for clients.

Skills
Skills & Competencies

Strong computer skills and knowledge of Microsoft Office (e.g. Word, Outlook, Excel, etc.).
Strong written and verbal communication skills.
Strong interpersonal skills.
Strong coordination and teamwork skills.
Foreign languages a plus.

Notes
Qualifications, Education and Experience

Bachelor’s Degree required. MSW or MA in closely related field preferred.
Minimum of two years of experience in case management, individual or group counseling preferred.   
Experience with developing and running psycho-educational groups preferred.
Knowledge of supportive/ supported housing, Section 8, and NYC housing subsidy programs preferred.
Experience with homeless, mentally-ill and substance abuse population preferred.
