Job Summary
Reporting to the Program Coordinator, the Clinical Social Worker provides direct care and guidance, ensuring evidenced based practices are being utilized to expedite a person’s recovery. The Clinical Social Worker will be actively involved in crisis planning and working to reduce the number of people on the high acuity list by deescalating situations and taking a preventative approach when signs are observed early. In addition to crisis work, the Clinical Social Worker will serve as an interim provider, providing individual and group counseling to people who are not ready to be connected to more formal care (i.e. clinics, traditional ambulatory treatment teams, etc).

Key Responsibilities & Essential Functions

Program/Facilities
Assists to ensure that performance-based incentives and all other goals of the program are met.
Provides clinical expertise to program staff
Housing documentation and placement
Medical and mental health services
Completes clinical provisions of the program for people in our care, including but not limited to care plans, housing placements, substance use counseling, incident reporting, and all other activities
May coordinate clinical meetings with staff and other inter-disciplinary teams.
Helps with liaison and coordination of services with psychiatric and medical providers.
Is on call to address emergency needs on a twenty-four hour basis.

Administrative
Prepares, maintains and submits timely and accurate statistical and other reports mandated by Urban Pathways, regulatory and funding agencies.
Assesses, coordinates and facilitates staff Training.
Provides back-up administrative coverage in the absence of the Program Coordinator as needed.
Completes, when needed, psychiatric evaluations. (e.g. An individual’s housing package is incomplete because they are not able to get a psychiatric evaluation or refusing to go to an outside provider, the Clinical Social Worker will meet with the person and complete an evaluation with them.
Collaborates with the Program Coordinator and Human Resources to recruit, hire and onboard new program staff.
Manges AWARDS and other systems to ensure that all necessary client data is accurate and timely.
Ensures paper and electronic charts comply with Urban Pathways and regulatory agency standards.
Conducts monthly group supervision and study groups for employees interested in obtaining licensure.
Assist with agency-wide trainings to help develop our staff.
Utilizing a new admission’s chart in Awards, the Clinical Social Worker reviews their assessments to determine their eligibility for RTS services. If deemed appropriate, an LPHA form will be completed within 20 days of the person’s admission.

Community Relations
Develops and maintains ongoing relationships with community services and resources. These relationships will assist linking people to community referrals to ensure the person’s whole health is being addressed.

Skills
Qualifications, Education and Experience
Master’s degree in Social Work or closely related clinical field required.
Minimum five years of clinical and supervisory experience required.
LCSW required.
Experience with people experiencing homeless, mental illness and/or substance use population preferred.
