Come join our team as Family Liaison/Social Worker—reporting to the Executive Director, interacting personally and directly with families, coordinating with the medical and support teams to match needs with services available, engaging with the Board to strategize and develop new ideas, being part of a small, focused, passionate team seeking to do good.

ABOUT US
The Making Headway Foundation is a non-profit organization that provides a variety of services and support to families impacted by childhood brain or spinal cord tumors. Making Headway has a close working relationship with NYU Langone Hospital, as well as other Tri-State area hospitals. The Family Liaison/Social Worker works directly with families, coordinating care and services both in and out of the hospital. All applicants must be a licensed social worker (LSW) or a licensed clinical social worker (LCSW) and have a caring and compassionate personality, an interest in engaging with a diverse team to find effective solutions, and a dedication to helping those in need.

The Making Headway office is located in Chappaqua, NY. The Family Liaison/Social Worker is a full-time position, with a starting salary of $70,000-$100,000 depending on qualifications and experience. Position includes Medical and Dental.

POSITION DESCRIPTION
Outreach and Follow-Up
Contact new families within 36 hours of a referral to assess their needs. Follow-up as needed with all families that reach out for assistance or are currently in treatment or crisis. Work with social workers at regional hospitals to identify new referrals.

Case Management
Provide information/resources/assistance as needed. These services may be very varied and include anything from Making Headway programs, state-funded programs, hospital-based services, or programs offered by other non-profits. Assist in obtaining resources for unmet family needs, such as housing, financial support, or technology.

Case and Ongoing Care Team Coordination
Coordinate care by networking with hospital social workers and medical teams to understand what services and support are necessary. Based on the family’s needs, determine and coordinate referrals to psychiatrists, psychologists, and educational consultants on the Making Headway Ongoing Care Team.

Manage and Maintain Database                
Develop, update, and maintain family database—including detailed records regarding care and support needed, contact information, and event participation.

Other
•        Meetings with hospital staff and medical teams to discuss services Making Headway provides, as well as occasional meetings with new families at the hospital (NYU Langone)
•        Attend Making Headway Family Events
•        Develop and coordinate local volunteer base
•        Create/conduct needs assessment survey for families
•        Update and compile “Resources Package” for new families
•        Occasional availability on nights and weekends to respond to families with emergency needs
•        Other tasks, as needed.

Skills
•        Expertise in a variety of social work fields, including psycho-social focus on children and families, medical, and/or mental health
•        Experience in crisis situations
•        Knowledge of trauma and secondary trauma helpful
•        LSW or LCSW (NY State preferred)
•        Computer skills: Microsoft Word, Power Point, and Excel
•        Spanish/Mandarin/Russian speaking a plus

Limited Hybrid Options and Flexible Days
Flexible schedule for evening and weekend outreach possible. Remote days possible after six months. Normal office hours are 9am-5pm, Monday-Friday.
