Description
Title:   Residential Staff LPN 
Schedule: Monday – Friday. 12p – 8pm, or 1pm – 9pm Shifts available. 
Salary: $70,000
*Bonus weekend shifts available at flat shift rate* 

Position Overview:

Reporting to the Assistant Director of Nursing Embedded Services for nursing clinical related supervisions. Indirectly reporting to the Director of Nursing Operations (DNO). Reports to the Program Director of designated residence for client assignments, daily tasks, and workshop scheduling. The Residential Staff LPN will provide educational resources and workshops for chronic health conditions and serve as a health promotion agent to improve health outcomes within transitional and permanent housing programs.

Essential Duties and Responsibilities:           

Assist staff and residents to understand medication regiments, medication management, and assist with ensuring compliance with medication adherence
Provide informational workshops to promote health awareness and education workshops to residents to support improving health outcomes
Perform quality assurance inspections of medication storage, medication bottles, pill boxes, and dispose of expired medications
Perform site visits to resident units to evaluate opportunities for teaching, coaching and education pertaining to health and wellness
Assist with outreach to healthcare providers regarding refills and other medication/medication related issues
Assist residents to perform daily vital signs as needed; coordinating care with primary care as needed
Assist with crisis intervention, such as calling 911 for emergency room transfer if emergency psychiatric or medical evaluation is required and ensuring connections are made to providers and leadership as  needed
Document patient encounters in eCW as required
Communicate with treatment providers and case management team regarding resident’s clinical status as needed.
Administer IM injections or insulin subcutaneous injections as prescribed, on an as needed basis.
Participate in weekly clinical team meetings and multidisciplinary case conferences to discuss high risk clients and  to discuss health concerns, medication changes, hospital discharge follow-up on behalf of residents         
Other duties as assigned

Skills
Education Credentials/Requirements:
New York State Licensed Practical Nurse (LPN)
Experience
Prior experience working with homeless individuals preferred
3-5 years of clinical experience in residential care
Skills:
Communication:     Must have excellent verbal and listening skills. 
Interpersonal skills: Must have the ability to interface with clients as well as all levels of staff.
Must have knowledge Microsoft Office, especially Word and Excel. Familiarity with eClinicalWorks preferr
