This position offers an exciting opportunity to have a positive impact on the Sunset Park, Brooklyn community through engaging with people through AMPHS program development and direct service delivery. Your role will be pivotal in delivering exceptional clinical support to our clients, helping them navigate through various challenges and ensuring their overall well-being.

In this team, the Social Worker is responsible for conducting in-depth health risk assessments covering social, physical, and environmental concerns for adults and families. S/he will identify the client’s needs and provide follow-up to connect them to available community resources. Engagements will focus on addressing barriers to consistent care, resources, and treatment.
Other responsibilities include, but are not limited to:
•        Managing a regularly assigned caseload of community members using various approaches, motivational interviewing, and goal setting, while evaluating progress and modifying treatment plans accordingly.
•        Engaging in short-term interventions through the coordination of referrals and services; Utilizing our network of community partners to advocate for clients and refer to specialized services.
•        Developing and facilitating educational workshops and support groups.
•        Providing direct counseling during seasonal community screening events.
•        Ensuring that knowledge gained through the community is fed back to AMPHS in a manner that informs current community needs and future programming.

The successful candidate will be knowledgeable of the healthcare system, have a deep understanding of the psychosocial aspects of illness and health, and superb interpersonal and communication skills.
They will also have a strong understanding empowering individual agency and choice, experience in case management, crisis intervention, and mental/behavioral health support.

Minimum Requirements:
•        Current NYS LCSW or LMSW license required.
•        1-2 years of experience as a Social Worker in a community-based organization.
•        Computer literacy: Solid command of MS Office; Experience with Electronic Medical Records (EMR) or client record databases a huge plus.
•        Bilingual (Spanish/Mandarin (Chinese) a highly preferred.

Starting Salary: $60,000
