Non-ExemptPATH is seeking experience licensed social workers (LCSW, LMFT, or LPCC) eager to take a leadership role in making a positive impact in the lives of others to join our Veteran's Interim Housing team in West LA as a Clinical Supervisor.
WHAT IS A CLINICAL SUPERVISOR?
As part of the Interim Housing team, the Clinical Supervisor provides direct support to the Program Manager as well to their case management colleagues. Responsibilities include:
Provide clinical support, training and supervision to program staff including clinical supervision for Board of Behavioral Services registered ASW, AMFT, and APCC providers accruing hours toward licensure.
This position is expected to be available for clinical consultation and regular supervision. Be knowledgeable and have experience providing evidenced based practices effective when serving a vulnerable homeless population including but not limited to, Motivational Interviewing, Critical Time Interventions, Harm Reduction, Housing First and Crisis and Suicide Assessment.
Process and accept referrals from funders
Meet with assigned veterans a minimum of 1x/week to support with progression towards goals identified by guest in their ISP
Facilitate psychosocial rehabilitation groups for veterans as required
Conduct intake and individualized needs assessment for all veterans and assist team by collaborating with veterans to develop Individualized Service Plans (ISP) that address barriers to obtaining services, housing, and appropriate health care needs
Conduct a full VI-SPDAT (Vulnerability Index- Service Prioritization Decisions Assistance Tool) at admission for all new veterans to aid in the Individual Service Plan (ISP) development
Conduct and complete all new veterans intakes and enter information into HMIS in a timely fashion
Provide ongoing case management support with benefits establishment (GR, SSI, TANF, Veterans pension, etc.), obtaining ID, birth certificate, social security cards, provide employment linkages and assistance, housing referrals and health related assistance as identified on the ISP
Provide referrals and linkages to primary and behavioral health services and other supportive services
Collaborate with referring organizations to coordinate veterans’ care and ensure veterans receive the services and support they need to achieve and maintain health, mental health, and housing stability
MANDATORY REQUIREMENTS
Have employment eligibility verification
Ability to work flexible hours which may include evenings or weekends
Have or be able and willing to obtain CPR/First Aid training
Provide proof of full COVID-19 vaccination
