As part of the LIFE's Senior Services PACE (Program of All-Inclusive Care for the Elderly) team, we perform a vital service by providing primary care for Tulsa's aging population. Our amazing team of professional's access to comprehensive medical services and social support at the site while they enjoy independent living in their own homes and communities. Most participants have numerous chronic conditions and have had minimal healthcare in the past.
We are looking for a Master's Social Worker (MSW) to grow our staff and join a team of professionals, who excel in providing unique health care for Seniors! The MSW's main role is find connections and facilitate collaborations between PACE participants, caregivers and community resources to help meet the needs of the participant. This role gives you the opportunity for interdisciplinary team collaboration working closely with physical therapists, providers, social workers, and nursing staff to ensure the best for our participants. Your role is assisting with finding the right resources to address an unmet need of the participant; these needs may include, housing, mental health, support for caregivers. This role is ever evolving as we are always addressing new needs presented to us by of our participants and their families. In addition, you will participate in regular assessments and review care plans with the interdisciplinary team. Since you will be working exclusively with seniors, some with dementia, patience and extra compassion will sometimes be required. You will have the opportunity to build a long-term connection and relationship with the participants and their families.
Education and Experience:
•Master's degree in social work,
•Experience in working with a diverse, elderly population, preferable in a community-based setting or geriatric program.
•Good communication skills to effectively communicate with participants and families
