Description
Catholic Charities Kingsborough Transition Team (Pathway Home) works with clients transitioning to the community from a higher level of care. This multi-disciplinary team addresses the preparatory needs of clients in the early stages of recovery, discharge planning, development of daily living skills, and coping mechanisms through team and individual work in the community. The team will follow individuals into the community using the Pathway Home model of care to ensure that they become both linked and engaged with identified community providers. The team delivers comprehensive, community-based services for adults living with serious mental illness, and ensures clients have access to uninterrupted and coordinated behavioral and physical health services while addressing the social determinants of health that impact daily living. The Nurse will address a host of issues that impact clients directly such as housing, access to nutritious food, economic security/benefits, medication adherence, linkage with outpatient treatment providers or other community resources and social supports. This position is part of an embedded team at Kingsborough State Psychiatric Center. RESPONSIBILITIES: The Nurse role involves program planning and development, community-based assessment and treatment of clients, and crisis management. The Nurse will perform a wide variety of tasks related to the care, support, education, socialization, habilitation, recovery/healing, safety, security, and support of adults diagnosed with mental illness and substance use disorders who are transitioning to various levels of independent housing settings. The Nurse is responsible for the diagnosis and treatment of human responses to actual or potential health problems through such services as case finding, health teaching and counseling. The Nurse will provide interventions supportive to or restorative of life and well-being; and execute medical regimens prescribed by a physician, PNP or another licensed health care provider. Duties of the Nurse focus on assessing client health problems reflective of holistic needs, developing and implementing nursing care plans, and completing health assessments. The Nurse will consult and coordinate with healthcare team members to assess, plan, implement, or evaluate care plans. The Nurse will advise clients on health maintenance and disease prevention including effects of diet and physical activity and provide case management; provide health education and connection to primary care to address physical health issues that impact a client’s ability to successfully live in the community; and provide health education for patients and their families for specific medical needs, preventative care, and how to access and utilize healthcare services in the community. Based on a comprehensive review of the client's overall condition, the nurse will continually review and evaluate plans of care and nursing interventions to ensure that such interventions are consistent with the client's comprehensive treatment plan. The Nurse is also responsible for providing direct treatment and support services to clients with psychiatric diagnoses, co-occurring substance use, chronic physical health problems and complex social needs. Service provision takes place at inpatient settings, in the clients’ home environment, community settings, provider offices and additional locations as needed.
