Description
-Assist assigned Caseworkers in completing all items on the screening checklist; which consists of requesting termination of parenting rights for each child, birth certificates, death certificates, surrenders, service plans and social histories. -Worker is responsible for completing a complete adoption subsidy and the legal documents necessary. -Worker will prepare families for adoption and provide support. -Worker will formalize tools and techniques for the home study process .required for adoption/Guardianship -Worker will submit case closing information to Juvenile court and then prepare the case closing, subsidy within five working days of the finalized adoption. -Worker will complete subsidized guardianship packets and submit to DCFS. Worker will complete post adoption/Guardianship closing documents and submit to DCFS post adoption units .. -Worker will complete statistical reports for all children who have reached permanency during the month. -Worker will communicate all procedures of the cases to the Supervisor .and assigned Case Manager. -Worker will coordinate with Case Manager to ensure that biological parent has all needed resources to facilitate reunification with child(ren) in care. -Permanency must deliver independent living skills services to youth over the age of 17 in IDCFS care or link these youth to required services .The Permanency worker will document such services in SACWIS as required. -The Permanency Worker will ensure youth turning 17 years of age complete the CFS 2030-1 NYTD Baseline Survey within 45 days of his/her 17th birthday and every third year after if he/she is still under IDCFS Guardianship. -Participates in regular Supervision and team meetings as required.
