Housing Coordinator

-            Conduct weekly or bi-weekly meeting with clients to continue the housing procurement, financial, and self-sufficiency case management plans. Preparing progress notes and bio-psychosocial histories as per agency standards when the package expires. Complete case notes in a timely manner (48 hours) documenting the reason for the meeting, the occurrence during the meeting and the plan for the next meeting.
-        Provide intervention and advocacy with landlords and housing providers on the client’s behalf to develop a workable plan to obtain and or maintain housing.
-        Assist participants in locating and securing appropriate housing. Complete housing appointment slips and appropriate case notes in a timely manner.
-        Networking with community housing providers to coordinate appropriate housing and provide appropriate referrals. Conduct monthly visits to form relationships with additional landlords, brokers and housing providers utilizing the in-house spreadsheet.
-        Data entry and data retrieval from FileMaker database and CARES.
-        Participating in weekly Case Conference as needed
-    Facilitate two groups a week pertaining to housing and maintain housing. Provide updated syllabus for both groups.
-        Maintaining accurate charts as per agency standard.
-        Complete daily, weekly and monthly reports of placements as needed. Utilizing FileMaker Pro, CARES and Excel databases.
-        Escorting clients to housing appointments and other travel arrangements.
-        Develop and maintain a housing data-base and present the updated version during weekly supervision.
-        Submission of housing packages and follow-ups with Department of Homeless Services   in a timely manner.
-        Meets regularly with the immediate supervisor as a means of enhancing professional growth, reviewing and processing the provision of case management services, and dealing with appropriate administrative issues.
-        Assist with intake assessments as needed.
-        Attend all staff meetings with a mutual goal of contributing to program issues and updates on agency issues.
-        Complete and email the appropriate to the appropriate parties the weekly veteran’s tracker with all appropriate updates.
-         Complete TAP request CARES, letter and application and email to the appropriate parties.

Skills
Must have a strong, verbal, written and communication skills. Able to negotiate with landlords/brokers and willing to escort clients for viewing of the units. Must have great organizational skills

Notes
General hours are Monday - Friday 11a - 7p
Must be available one weekend a month.
