Description - Maintaining accurate charts as per agency standard. - Networking with community health care providers to coordinate care and provide appropriate referrals - Providing some crisis intervention as needed - Complete daily, weekly and monthly reports and assessments of clients utilizing FileMaker Pro, CARES and Excel databases. - Meets regularly with the immediate supervisor as a means of enhancing professional growth, reviewing and processing the provision of case management services, and dealing with appropriate administrative issues. - Attend all staff meetings with a mutual goal of contributing to program issues and updates on agency issues. - Assist with intake assessments as needed. - Supervises and provides leadership and training to social service team members. - Supervises day to day operations and clinical staff in the absence of the Program Director. - Assist with conducting case conferences and resolving client issues. - Manage all incident reports and client grievances ensuring the appropriate staff has a copy for the client’s file, schedule meeting as appropriate and work on resolving the issues. - Participates in external meetings with funding agency, community service providers and housing providers. - Assist with intake assessments as needed. - Other duties as needed.
Skills The individual must have the following -BA in the field of Social Services [MSW preferred] -At least five years' experience in supervision chart review. -At least five years' experience and knowledge of supervision with MICA, Housing, Social Worker; and Case Manager. -Must have excellent written and verbal skills -Excellent communication with all levels of staffing. Must have strong organizational skills
Notes Work hours are general Monday - Friday 11a - 7p Must be able to work one weekend a month
