Under the supervision of the TANF Program Manager or his/her designee, the Housing Stabilization Case Manager is responsible for assisting families approved for the Temporary Assistance for Needy Families (TANF) Housing grant, award to HASLC with funding through June 2025. Families on this program receive short-term rental assistance, such as rent payments, security deposits, and application fees. This position provides short-term individualized case management services to families once they are housed in order to maintain stable housing and avoid returning to homelessness.

Essential Functions
1.        Provides case management services to families enrolled in TANF Family Housing Program, including completing required forms, setting goals, creating budgets, providing employment leads, and making appropriate referrals.
2.        Responds to clients, providers and community. Maintain client files in accordance with required guidelines.
3.        Regularly document meetings in Homeless Management Information System (HMIS) in a timely manner.
4.        Meets with participants on a regular basis to review program progress. Perform exit assessments accurately for clients leaving program.
5.        Assist with audits, both internal and external.
6.        Develop and maintain productive working relationships with partnering agencies, outside organizations, landlords, and service providers in Salt Lake County.
7.        Performs other duties of a similar or related level as necessary or assigned.

•        Maintain and enhance linages and partnerships with various community partners in order to build effective collaboration.
•        Proven ability to work as a team and independently, accurately and detailed in an atmosphere of frequent interruptions.
•        Exceptional organizational skills, attention to detail, time-management skills, and motivation to meet deadlines and achieve goals.
•        Ability to communicate effectively both orally and in writing. Ability to relate and communicate with persons from all socio-economic, ethnic and racial backgrounds.
•        Ability to learn, follow and apply a system of complex regulations, policies and procedures.
•        Ability to maintain confidentiality.
•        Proficient in Microsoft Office Suite and related software. Ability to create computerized reports and submit information to grant providers.
•        Must have reliable transportation. Valid Utah driver's license and insurable with HASLC on a continuous basis.
Education & Experience

Bachelor’s degree in Social Work or related field with at least two years of case management, social work, or similar; or an equivalent combination of education and experience.
