Description
Under the supervision of Program Manager, in accordance with the standards of the social work profession and requirements of the funding source, the PEARLS Therapist/ Licensed Master Social Worker will:Determine program eligibility for individuals referred to the PEARLS program for treatment of depression and/or depressive symptoms. Provide in-home or telehealth treatment visits on a full time basis, using the PEARLS’ Problem Solving Treatment & Behavioral Activation intervention models to an identified caseload of older adult clients who could benefit from depression treatment. Administer PHQ-9 depression questionnaire and other mental health screening tools to clients so as to screen for possible disorders and impairments requiring mental health services. Attend DOHMH approved PEARLS training and complete online refresher training annually thereafter. Participate in group clinical supervision with the program’s psychiatrist/ clinical supervisor interdisciplinary team conferences, reviewing treatment plans, case monitoring and discharge planning, pursuant to sound professional practice and to federal, state and city guidelines. Represent the agency at specific DOHMH events and meetings as well as other community conferences and meetings. As appropriate, make referrals to and maintain cooperative relationships with public and voluntary agencies, programs, services and facilities. Participate in training and other unit meetings. Complete required case records, reports and statistics within mandated timeframes, utilizing computers (Netsmart EHR, NYCMED, Microsoft Excel, etc.). Perform other duties as assigned.

Skills
Possession of a New York State License in Social Work or mental health counseling is required. Good writing/record keeping skills. Ability to read, write, speak, and understand English. Must be computer literate, including ability to learn and use program-specific software.

Notes
Ability to travel to clients’ homes and to negotiate any steps and barriers in clients’ residences. Knowledge of Spanish will be helpful.
