Description
In accord with the standards of the social work profession and requirements of the funding
source, the Program Director:
? Oversees the day-to-day operations of the social and recreational program for members
with dementia, including the development and implementation of program curriculum
and activities.
? Coordinates special programs and events for program participants, including field trips.
? Performs intake interviews with participants and family members, screens for program
eligibility, and completes all necessary application paperwork.
? Conducts a comprehensive, in-home assessment of those members deemed eligible for
the program to both plan for the members’ participation and to identify and address other
unmet needs.
? Develops an initial care plan based on the assessment, provides ongoing case
management related to the implementation and monitoring of the care plan, and
performs regular reassessments.
? Recruits, trains and directs Program Assistant, Volunteers, and Consultants in the
provision of services to the program participants.
? Maintains statistical and financial records for the purpose of monitoring income and
expenses of the program.
? Prepares and submits required reports to funding sources and all authorized and
interested parties.
? Prepares bills to caregivers for fee/contribution collection.
? Provides staff and community education, as well as facilitates caregiver support groups.
? Serves as advocate on behalf of clients’ families and collaterals, both internally and
externally to JASA.
? Perform such other duties as assigned.


Skills
Graduate of an accredited school of Social Work with MSW or equivalent Masters-level
degree
? Demonstrated organizational skills and ability to provide professional supervision and
leadership to staff.
? Minimum of two years of experience in social work or related profession.
? Ability to read, write, speak, and understand English.


Notes
?Computer-literacy, with ability to learn program-specific software. Should be conversant
with current Microsoft and Google applications.
? Excellent writing and record keeping skills.
? Understanding and appreciation of the roles of race, religion, ethnicity, sexual
preference, and individual values as they relate to serving clients and families.
