Specific Education, Certification and Skill Requirements (if applicable): Masters degree in Public Health, Social Work or any closely related field. At least two years of experience working with or leading a chronic disease collaborative. Experience managing amd leading a health care coalition a plus. Must have excellent writing and communication skills and have the ability to work in a tean environment.
Job Description: Northern Manhattan Perinatal Partnership (NMPP) a maternal and child health agency located in Central Harlem seeks a Program Manager for its Building Bridges, Building Knowledge, Building Health Coalition (BBKH). The manager will function as a liason with all coalition members. Coordinate with CBOs,local pharmacies and community pysicians to inform them of workshops. Assume leadership in the social marketing initiative to assist with impoved outcomes of people with diabetes. Assist coalition members in the identification of distribution sites for the dissemination of information to schools, housing projects and churches. Organize meetings, ensure minutes are documented and subcomittees meet. Work with the Executive Director of NMPP in gathering and analyzing consumer feedback to determine campaign effectiveness and identify opportunities for improvement. Build relationships with other organizations and community health networks. Establish BBKH Diabetes Health Disparity Task Force, sharing best practices and new research interventions. Collaborate with BBKH Coalition members in conducting external and internal analysis of the environment, decide and agree on strategies to coordinate diabetes care in the region. Develop monthly, quarterly, annual and final reports to BBKH leadership. Perform other duties as required for the coalition.
