Specific Education, Certification and Skill Requirements (if applicable): BA in human services/related and QMRP qualified. Must have 3+ years experience/training in field of human services/working with people with DD. Proficiency in computer skills, valid driver's license and personal vehicle also required.
Job Description: Manage caseload of approx. 14 adults with DD. Complete QMRP paperwork and coordinates staffings. Solicit ideas, write, and train in Personal Outcomes. Ensure human rights and safety assurances, completes reports and performs OIG investigations. Develop positive behavior support plans and coordinates outings. Accompanies clients to psych appointments. Cover on-call pager once a month and work one weekend per month
